If anyone should know, it's Dr. Stamey:

“The PSA era is over in the United States,” said Thomas Stamey, MD, professor of urology and lead author of a study published in the October issue of the Journal of Urology, which was covered by CBS Evening News and Reuters among other outlets. “Our study raises a very serious question of whether a man should even use the PSA test for prostate cancer screening any more.”

The findings reflect a shift in Stamey’s thinking from 1987, when he published the original findings in the New England Journal of Medicine that linked increased blood PSA levels to prostate cancer. Over the years, Stamey has come to believe that the PSA test is no longer a useful predictor of the amount or severity of prostate cancer. He said elevated PSA levels actually reflect a condition called benign prostatic hyperplasia, a harmless increase in prostate size.

Stamey explained that the change in his thinking is due to the increased screening for prostate cancer. Now that screening is more commonplace in this country, many cancers are being caught earlier and are usually smaller – not generating enough PSA to be a good indicator of severity. By contrast, he said, the tumors encountered 20 years ago were generally so large that they generated PSA levels high enough to provide a reasonably good measure of cancer severity.



What Men Need To Know About Prostate Cancer and PSA


Prostate cancer is the second most commonly diagnosed type of cancer in men, with lung cancer being the first. Luckily, prostate cancer also happens to be a type of cancer that responds extremely well to alternative cancer treatments. These misunderstandings should have been challenged and abandoned decades ago. But since they weren’t, men with prostate cancer may need to educate themselves and gain a better understanding than their doctors, if they are to save their lives.

For most of the last century, prostate cancer has occurred primarily in older men and was long known for being slow growing in most cases -- so slow-growing in fact that men over 65 could often simply live with their cancer without using any treatment at all. This was because they had a good chance of dying of old age before dying from their cancer.


Understanding Gleason Grading

Physicians use the term "grade" to describe the appearance of thin slices of cancer tissue when it is observed under a microscope. In the case of prostate cancer tissue, the most common system used in the USA to grade the appearance of this tissue is called the Gleason grading system, after the physician who first described this system.

The Gleason grading system is not the only grading system in use around the world. However, because it is the one most commonly used in the USA, it is the one we will try to explain here. If your physician talks to you about the grade of your prostate cancer, you may want to ask if it is the Gleason grade that he or she is referring to.

The Gleason grade is one of several pieces of information determined by the pathologist who examines the biopsy specimen taken from the prostate. Readers wishing to understand more about the role of the pathologist in the diagnosis of prostate cancer may wish to read the article entitled "The pathologic examination of prostate tissue," which addresses this topic in detail, and which also explains the importance to the patient of receiving a copy of his pathology report.

The Gleason system is based exclusively on the architectural pattern of the glands of the prostate tumor. It evaluates how effectively the cells of any particular cancer are able to structure themselves into glands resembling those of the normal prostate. The ability of a tumor to mimic normal gland architecture is called its differentiation, and experience has shown that a tumor whose structure is nearly normal (well-differentiated) will probably have a biological behavior relatively close to normal -- that is not very aggressively malignant.

Selenium, Brazil Nuts and Prostate Cancer

In a new finding, high blood levels of the mineral selenium were associated with a four- to fivefold decrease in the risk of prostate cancer. Scientists at Stanford University suggest taking selenium supplements to ward off this most common form of cancer in men.

The California researchers studied 52 men who had prostate cancer and compared them to 96 men who didn't. Blood plasma levels of selenium were measured for years before any diagnosis of prostate cancer, according to a report in the December issue of The Journal of Urology {2001;166:2034-8}. 

One surprising finding was that blood levels of selenium generally decreased with age. As is well known, the risk of prostate cancer increases dramatically as one ages.

"This study showed that there was a direct connection between selenium and prostate cancer," said Dr. James Brooks, lead author of the study, in a university news release. "Older men with higher levels of selenium were at lower risk, while, conversely, "individuals with the lowest range of plasma selenium represent a population at risk for the development of prostate cancer."

"These results support the hypothesis that supplemental selenium may reduce the risk of prostate cancer," said Brooks. Other studies have shown that selenium is associated with a reduction in the risk of other cancers as well, including those in women.

The latest findings are dramatic. Selenium is easy to get and take, while prostate cancer can be devastating. But notice something else: in the past, important findings in the field of prevention were almost always followed by weasel-worded statements, such as "scientists do not recommend that people actually take food supplements."
